Please
be advised the Benefits Office has a resolution for the Tier II members who have
had their PSP accounts cashed out. Information
that was forward to us from Tier II member, Ben Milligan was used to present
this problem to the UAW International. The end result is that every Tier II
member who returned to work within 5 years of separation will have their PSP
account restored. We also found out that this is Corporate wide and not an only
an Orion issue. Members are asked to be patient while a system is set up to
acknowledge all who have been affected. Thanks to Ben Milligan for his efforts
in helping us solve this issue.

FROM THE BENEFITS OFFICE:
Attention all Tier 2 members who are trying to receive sub, you need to
do the following:
1)
Print out your full payment history from www.michigan.gov/uia
2) Call your Michigan Works branch and get a
statement explaining the status of your
TRA
and/or how many weeks are left on the
TRA
(Monetary Determination) Mondet.
3)
Fill out a Multiple Week Application (available in the Benefits Office or at www.layoffbenefits.com)
4)
Fill out Dependent Information (DI) form (available in the Benefits Office or at
www.layoffbenefits.com)
After you have all your documents
bring them to the benefits office or fax them to 248-377-5390.
Write under the subject “Tier 2
SUB
.”
Please include your phone number on the cover sheet. Thank
You.

Debit
Cards:
The
UAW Local 5960 Transition Center has been informed that the Unemployment Debit
Card, that you were issued, is good for three (3) years. If you are a
laid-off Skilled Trades Person who was laid-off in November of 2009, you should
continue to use your existing Debit Card. Call Chase Bank at
866-523-2122 to report lost or stolen Debit Cards.

Profit
Sharing:
The formula for profit sharing can be found on page 20-21 in the “Know Your
Benefits” booklet recently mailed out to the membership. This formula is used
to calculate a portion of the profit based on your “Compensated Hours” in
2010. Employees are not credited for hours while on a layoff.
You have to be actively working. Tier
1 members who worked as Temps or Flex employees at other locations will receive
Profit Sharing based on the hours worked while at those locations. Your share of
any Profit Sharing distribution is based on your eligible compensated hours, up
to a maximum of 1,850 hours per year. Those hours generally include any time for
which you receive pay, including your straight time hours, for such things as:
* Bereavement Pay; * Call-in
Pay; * Holiday Pay; * Jury Duty;
* Overtime; * Short-term
Military Duty; * Vacation. You may choose to take your profit share as a
cash payment. If you elect cash, you will receive your profit share no later
than the third month following the plan year in which a profit share is
generated.
Profit Sharing Choices:
As
an alternative, when your profit share is above the $50.00 minimum payment
amount, you may direct the Company to place up to 100% of your share, in 1%
increments, into the Personal Savings Plan (PSP).
If you do, your money will be invested in your current PSP investment
options. You should refer to the first section of this booklet for information
on your Personal Savings Plan investment options and distribution procedures. If
you elect to place your Profit Sharing amount in the Personal Savings Plan, it
will be subject to Social Security taxes. But federal income taxes — and in
most cases, state and local income taxes — will generally not apply until you
withdraw your money at a later date. If you choose to save your Profit
Sharing amount in the Personal Savings Plan, any taxes (e.g., Social Security
taxes) will be withheld from your next regular paycheck — not from your Profit
Sharing amount. Whatever your Profit Sharing choice, remember that your
election will remain continuously in effect until you change it. To change
your percentage going to your PSP you have to contact PSP
at 1-800-489-4646, DO NOT call the Benefits Office to change your PSP, That
has to be handled by you on a recorded line. Profit Sharing Checks will be

(These steps also apply when you
re-certify for SUB Benefits) When members exhaust regular
unemployment benefits they transition into EUC. It is at this point
SUB
can no longer see that you are being paid unemployment. Members will have to
submit the following documents to
SUB
:
1.
A detailed billing history with your Monetary Determination for
EUC -OR-
A printout of your payment history from UIA website with the weeks
paid EUC (Preferred) 2.
An application for either
SUB
or
TSP
filled out correctly. (www.layoffbenefits.com
under GM forms) Once you have all your documents they need to be
faxed in TOGETHER to
SUB
Office Attention Michigan Analyst at 248-365-9809. If you do not fax the
documents in together it will be considered incomplete and will not be
processed. If you do not send it Attention Michigan Analyst, it will take longer
to process while they try to locate what state you belong to.
The unemployment office is not as cooperative with the process as they
have been in the past. This is due to the fact Michigan has a website where this
information can be obtained by the membership themselves. It is becoming the
member’s responsibility to take care of their own business. Members need to
register at www.michigan.gov/uia,
click the “Online services for unemployed workers” link. Members who
do not have a computer; this will no longer be an excuse. The Transition Center
and Benefits office will assist members to register on this website. You can
also use the computers at your local libraries or find a family member or friend
to help you set up an account. There are free email sites like Hotmail and
AOL
that you can register with for an email account. Once you are registered
on the website you will be issued a temporary password. The temporary password
does not allow you to access the payment history portion. You will receive a
password in the mail in about a week. The
new password will allow full access to the account.

Relocation
Money:
Employees
relocating to another state might
qualify for an extra $1500.00 in relocation money from Michigan Works but you
must apply prior to
moving. Contact your TAA
caseworker to see if you qualify.

Health
Care for 2011
Effective
January 1, 2011 Health Care Reform allows hourly active employees to add
eligible dependent children up to age 26 (until the end of the month in which
they turn age 26) to medical coverage only
(does not include dental or vision coverage).
All employees who are currently covering
dependents age 19 up to age 26 must verify eligibility. More
information will be mailed to your home in mid-November.
A dependent child may include a natural or adopted
son or daughter. Proof of relationship will be required for adult
children. Adult children are eligible for medical coverage regardless of
student, residency or marital status. However, an adult child is not
eligible for General Motors medical coverage if they are eligible for other
employer-sponsored group health care coverage. In addition, the spouse or
child of a covered adult child is not eligible for GM health care.
The enrollment period to add and verify eligibility
will be from November 17, 2010 through December 18, 2010.
Health Care Reform has no impact on the eligibility
rules under the Collective Bargaining Agreement for Dental or Vision. As a
reminder, employees who currently have or wish to add adult children (age 19-24)
enrolled in dental and/or vision coverage, must comply with the full-time
student rule. They must be enrolled as a full-time student at least one
semester in 2011. Documentation will be required. Further
information will be sent to your home early next year.
If the enrolled adult child is not
a full-time student , they may continue
enrollment in medical coverage only and the employee must attest that the adult
child is not eligible for other employer-sponsored group health care coverage.
If they are eligible for medical coverage with another employer, they are
not eligible for General Motors medical coverage and they will be cancelled
effective January 1, 2011.
More information will be mailed in mid November to
eligible hourly active employee homes explaining additional details and
enrollment instructions.

Life
Insurance Coverage:
Some
members have noticed Basic Life Insurance coming out of their
SUB
checks. This is standard to the contract for life insurance. The following
chart shows the continuance of Life Insurance based on years of seniority.

Please
note that seniority is NOT credited service. The life insurance chart will
go off your corporate seniority date. You can cancel your basic life insurance
by contacting Met Life at 1-888-543-3461. If you cancel your basic life you will
be enrolled again once you return to work. If you cancel any additional life
insurance (i.e. dependant, personal, optional), you will have to re-enroll once
you have returned to work. When you re-enroll you will start over at the lowest
schedule of benefits. If you wish to have more life insurance you will be
required to provide a bill of good health.

Sub
Update:
Once you get your TRA package we
will need the following to start your SUB:
1.TRA Monetary Determination (this will be in your
TRA package they send you)
2. We will need proof of your first TRA paystub or
web printout from www.michigan.gov/uia
with gross earnings no taxes taken
out.
3.Multiple week application for SUB/TSP from www.layoffbenefits.com
for the weeks paid TRA.
4. Fax in all these documents together to SUB, ATTN:
Michigan Analyst at 313-230-7492
Members are reminded that TRA is a form of
Unemployment and SUB will not pay out until proof of payment or denial has been
made to SUB. If you have not started your TRA process please call TRA at
1-866-241-0152. Please direct any questions concerning TRA to the number
listed here or to the Transition Center at 248-377-5049 or 248-377-5036.

Information:
Temporary
Employees: Last month’s Benefits Department article that appeared in the
5960 Report included a section about Temporary Assignment Employees. We have
recently received information regarding temporary employees. The Benefits Office
is now being told that temporary employees will get their
seniority when laid off from a temporary assignment, and health care will
be reset if the member worked 12 pay periods or more.
The Benefits’ Office would like to apologize for any concern this may
have caused.

TSP Payments:
Some of 5960 's younger seniority
employees will begin to transition to TSP as their SUB Benefits run out. Please
be aware that if you accept a lump sum from the TSP Program, you will sever all
ties with GM. To keep your GM Benefits and recall rights, opt for the weekly TSP
payments.
